Parent Support

• Parent Participation Letter - Give this letter to your employer to explain your need for a flexible schedule in order to work in the classroom. [MSW PDF]
• Fieldtrip Template - This template allows you to type up your child's name, address, and insurance information into a Word file which you then print onto the standard fieldtrip form. [MSW RTF]
• Request for Medication - This form allows you to send medication to school with your child. [JPEG image file]
• Newsletters - If you can't find that newsletter you know you stuck in your child's backpack, find back issues in the School section.
• Parent Donation Drive forms - Sign up to be a regular contributor to our school. [PDF]
• Donation Form - Use this form if you gave or solicited a tax-deductible donation of goods or money to Linscott. Fill out the form and bring it to the office for a signature. [PDF]
